New Patient and Client Registration
Our Practice Portal
All new patients and clients are required to sign up to our clinical portal called Function 365. This portal provides you with the tools to manage your appointments, liaise with our support team and upload or view your medical information including test results and care plans. Guidance on how to use this portal will be provided by our reception or clinic coordinator at the time of booking. Please refer to our FAQ document which provides helpful information and answers to general questions you might have.
As part of our Care Quality Commission (UK healthcare regulator) registration, and being a virtual healthcare service provider, it is a mandatory requirement for our clinic to verify our clients’ identification before proceeding with the appointment. We must ensure that the identity of an applicant for online services matches that of the individual who is registered at the practice and is formally recorded on our clinical system.
Identity verification protects both our clients and our practice from the consequences of fraudulent, accidental, or otherwise unauthorised access. To ensure that the practice can demonstrate that it meets its common law and statutory duties, it is important that the identity verification procedures are robust and that they are applied consistently. The following ID documents* will be accepted:
1. Copy of your passport
2. Copy of your driver's license
Once uploaded we will verify this information at your initial consultation.
*If you are unable to provide us with the requested photo ID, please notify our clinic coordinator or reception as soon as possible, so that we can look at alternative options to verify your identity.
Portal Actions Before Our First Consultation
Prior to your first consultation, you will be sent a confirmation email with the details of the appointment, any paperwork that needs to be completed and, in most cases, a link to complete your health and lifestyle history forms. Please note that this is an extensive form and you will need to allow around 30 minutes to complete it. Please have any supplements or medications you take to hand when you fill in the form and complete it as fully as possible. For anyone with a history of cancer, you will be asked for a copy of your oncology letters and any recent scan reports and blood test results.
You will need to complete and return the forms and any other associated paperwork , such as the 3-day nutrition and lifestyle diary, and upload your photo ID as above at least 2 working days prior to the appointment. It is crucial for our professionals to have the right information prior to seeing you in person, so that we can use our time together in the most effective way possible.
Please note that package appointments are payable before the consultations and single follow-ups are charged on the day. There is a 48-hour cancellation policy for all appointments.

Working With Us
1 Week before the Appointment
Please complete pre-consultation paperwork (intake forms, terms and conditions and information consent). Please share previous test results and medical letters with us and let us know who else is involved in your case.
At least 2 working days before the initial appointment
Please complete the 3-day nutrition and lifestyle diary and symptom scores via the portal. All paperwork must be received 2 working days before the appointment at the latest or the appointment may need to be rescheduled.
Prior to the appointment
You will receive an appointment reminder (please note our 48-hour cancellation policy). Please settle your invoice for packages as soon as possible and for individual consultations on the day of the appointment.
Following the appointment
Your written care plan will usually be sent to you within 5-10 working days*. Please book your follow-up as soon as possible to avoid delays and follow the testing guidelines in your plans. Please contact the clinic coordinator if you have any queries.
Follow-Up consultations
Our medical follow-ups are usually booked around every 6-8 weeks. Nutrition consultations are booked every 2-4 weeks depending on individual needs. Health coaching and yoga therapy often takes place weekly. You will be asked to complete symptom scores and nutrition and lifestyle diaries prior to every appointment to help track progress. Once you have achieved your goals, you will be provided with a health maintenance plan.
